Eric
M. Folkens, M.D. received two Bachelor
of Science degrees from the University of Louisiana
at Monroe. He completed a clinical laboratory scientist
internship at Tarleton State University and graduated
from medical school at Saba University School of Medicine.
Dr. Folkens served his internship
and residency in family medicine at Christus Spohn Memorial
Hospital in Texas and was the Chief Resident his last
year. He was the resident
representative for the Nueces County Medical Education
Foundation as well as for the Graduate Medical Education
Committee. Dr. Folkens is a member of the American Medical Association,
American Academy of Family Physicians, National Association
for Ambulatory Care, and the Manatee County Medical
Society. He currently has privileges at Manatee
Memorial as well Sarasota Memorial Hospital.
Dr. Folkens is a Board Certified Diplomat
by the American Board of Family Medicine and is licensed
to practice medicine in the U.S. Virgin Islands as well
as the state of Florida. He was in a group practice
before becoming a contract physician with several local
urgent cares.
Teri L. Folkens, R.N.
Teri Folkens received her LVN degree from Hill College in 1994. She received her R.N. from El Centro College in Dallas, TX in 1998. She currently holds nursing licenses in Texas and Florida.
During her 14 year nursing career, she has specialized in several areas which span from neo-natal nursery up to the charge nurse on a cardiac floor. Currently was employed at Manatee Memorial Hospital as a staff nurse working in several different areas of the hospital, but now works full-time at Bradenton Urgent Care and Lakewood Ranch Urgent Care.
Dr. Jan Gagel, M.D.
She was born in Louisville, Ky. and has two children who now live in Sarasota. She has a Masters in Nursing degree and practiced nursing for 10 years before going to medical school. She attended the University of Louisville School of Medicine and completed her residency in Family Medicine there as well. She is Board Certified in Family Medicine. She worked for the National Health Service Corps for four years providing healthcare for an area of Ky. that had a shortage of primary care physicians.
